Extra Support for your Patients with T1D
Blue Circle Health is a nonprofit organization funded by the Helmsley Charitable Trust that offers a free 6-month clinical care, education, and support program for adults living with type 1 diabetes. The services are offered as an extension of your care–keeping patients connected to their medical homes, and you updated. Today, Blue Circle Health serves 10 states—with more coming in 2025. T1D Exchange has opened a call for abstracts for the November Learning Session We invite QI Collaborative clinics to submit abstracts based on both T1D and T2D topics. Selected abstracts will be considered for publication in the Journal of Diabetes, as well as for oral or poster presentations at the November Learning Session.

T1DX-QI Coordinating Center New Team Member
Alyssa Cabrera, MPH, CPHQ, PMP, has recently rejoined T1D Exchange as Associate Director of Quality Programs. From 2016 to 2019, she was a key member of the organization’s Quality Improvement (QI) team. Most recently, she served as a Senior Project Manager at the Tufts Clinical and Translational Science Institute, where she applied her QI expertise to support researchers in enhancing the design and execution of their studies. Alyssa brings deep experience in Quality Improvement, Public Health, and project and program management—and as someone living with type 1 diabetes herself,
she is especially passionate about contributing to an organization whose mission resonates so strongly with her both personally and professionally. Outside of work, she enjoys building train sets with her almost three-year-old son, Rafa, and diving into the latest true crime documentaries.
QI Portal Users Feature
The T1DX-QI Portal has been adding features and enhancements over the years to assist you in your QI work. As a result, we thought it would be a fitting time to revisit the T1DX-QI Portal User Feature. What is your favorite feature in the QI Portal and why?
My favorite feature is the Compare feature since it allows us to both track our progress throughout the years but also look at the progress of other centers and see how we can improve! It is great to see how other centers are doing and to be able to use them as resources for our projects. If there is something that we see they are doing extremely well on, we can always reach out to each othe for feedback and support!
Practically speaking, which feature (in addition to the above) do you use most often and why?
Most often, we use the Dashboard feature as it easily allows us to view our centers metrics and compare this to the average metrics of the collaborative. The metrics are readily available and are referenced frequently while reporting or presenting our data internally. They really help us to see where else we can focus our QI efforts!
How does the QI Portal support your improvement efforts?
The QI portal is a fantastic tool which we use to see where we can improve and grow. The ability to share data, communicate with other centers, and track our progress is extremely valuable for all our QI efforts. In addition to this, being able to compare data with other centers and generate reports is very useful when presenting or reporting on our data.
What do you dream the QI Portal could do that it doesn’t do currently?
Our dream would be for the QI portal to see CGM and AID data, adjunct therapy data (such as that on GLP-1 agonists), and statin data.

Diabetes Education & Camping Opportunities
We'd love to see more kids with diabetes have the chance to participate in diabetes camp, so we are highlighting resources from ADA and New Venture Fund. Since 1949, the American Diabetes Association® (ADA) has provided children with diabetes access to adventure, education, and fun through camps and retreats led by licensed medical staff. ADA camps follow the highest standards for a safe and enjoyable experience. These camps empower children to develop essential skills, social connections, and a healthy lifestyle.
DECA aims to make diabetes camps inclusive for everyone. They are committed to welcoming all and staying informed on diversity, equity, and inclusion to ensure camps are for everyone. This commitment makes their camps and the world a better, stronger places.
